# Workplace Safety Assessment

Workplace safety audit form aligned with UK Health and Safety Executive
(HSE) standards, covering physical, chemical, biological, ergonomic,
and organizational risks, to identify hazards and verify control
measures in healthcare settings.

## Scoring

**Instrument:** HSE Workplace Safety Audit Checklist.

- **Compliant** — all controls in place, no action required.
- **Minor Findings** — low-risk gaps, action within 90 days.
- **Major Findings** — moderate-risk gaps, action within 30 days.
- **Critical Findings** — imminent risk, immediate corrective action
  and RIDDOR reporting if applicable.

## What it covers

Demographics and site details; PPE and hazard controls; chemical and
biological hazards; electrical safety; fire safety and emergency
egress; ergonomics and manual handling; emergency procedures; training
and competence; incident reporting and near misses; and sign-off with
an action plan.

## Common pitfalls

- **Treating the timescales as flexible guidance** — the 90/30-day
  windows for Minor/Major findings, and immediate action for Critical
  findings, are meant as firm commitments once findings are recorded,
  not aspirational targets.
- **Missing RIDDOR reporting obligations** — a Critical Finding can
  trigger a statutory RIDDOR reporting requirement; treating it as
  purely an internal action item can miss a legal obligation.
- **Auditing physical hazards while skipping organizational/
  psychosocial risk** — HSE's scope includes organizational risk factors,
  not just physical/chemical/biological ones; see
