# Vaccinations Assessment

Immunization-schedule compliance assessment covering childhood, adult,
travel, and occupational vaccinations, with contraindication screening,
consent capture, and administration record. Aligned with the UK Green
Book (Immunisation against infectious disease).

## Scoring

**Instrument:** Immunization Schedule Compliance (UK Green Book).

- **Compliant** — all age-appropriate routine vaccinations administered
  per schedule.
- **Partial** — one or more vaccines overdue or missing; catch-up
  schedule generated.
- **Non-compliant** — multiple routine vaccines missing or declined;
  clinician follow-up required.

## What it covers

Patient information; immunization history; childhood vaccinations;
adult vaccinations; travel vaccinations; occupational vaccinations;
contraindications and allergies; consent and information; administration
record; and clinical review.

## Common pitfalls

- **Treating all four vaccination categories (childhood, adult,
  travel, occupational) as covered by one blanket compliance check** —
  each has its own schedule and triggers; travel and occupational
  vaccination needs in particular are situational, not part of the
  routine baseline schedule.
- **Skipping the contraindications and allergies check before
  administration** — this is a direct safety step, not paperwork;
  proceeding to administration without it risks a genuinely preventable
  adverse event.
- **Using an outdated schedule reference** — the Green Book and national
  schedules are periodically updated; verify against the current
  edition rather than a cached or remembered schedule.
