Scoring

Instrument: Pre-Participation Physical Evaluation (5th ed.).

• Cleared — no concerns; full participation.
• Cleared with Conditions — participation permitted with named restrictions or accommodations.
• Not Cleared Pending Further Evaluation — temporary hold pending investigation (e.g. abnormal cardiac finding).
• Not Cleared for Sport — significant condition contraindicating participation in the chosen sport.

What it covers

Demographics; sport and position details; medical history (cardiovascular, concussion, asthma, musculoskeletal); family history (sudden cardiac death, inherited cardiac conditions); menstrual history/RED-S screening where applicable; cardiovascular screening (14-point AHA); musculoskeletal screening; neurological and concussion baseline (SCAT6); vision and skin; and the clearance decision.

Common pitfalls

• Skipping family history of sudden cardiac death — this is one of the highest-yield questions for catching inherited cardiac conditions that pose real risk during athletic exertion; it's easy to treat as routine boilerplate rather than the safety-critical item it is.
• Not establishing a concussion baseline before clearance — a SCAT6 baseline is what makes a future post-injury comparison meaningful; clearing an athlete without one removes a useful safety tool for later injury management.
• Treating "Cleared with Conditions" as equivalent to fully cleared — the named restrictions or accommodations are the point of that category and need to actually be communicated to and enforced by coaching staff.
