# Pediatric Assessment

Child development screening covering birth history, growth,
developmental milestones, immunization status, and family environment.

## Scoring

**Instrument:** Developmental Screen — Pass / Concern / Refer, based on
developmental milestone achievement, growth parameters, and
environmental risk factors.

## What it covers

Demographics; birth history; growth and nutrition; developmental
milestones; immunization status; medical history; current medications;
family history; and social/environmental factors.

## Common pitfalls

- **Screening milestones without age-correction for prematurity** — a
child born preterm should typically be assessed against corrected
age (not chronological age) for milestone expectations, especially in
  the first two years.
- **Treating "Pass" as meaning no further monitoring needed** —
developmental screening is meant to be repeated at scheduled
intervals; a pass at one visit doesn't exempt a child from later
  routine screening.
- **Missing the social/environmental context behind an apparent
developmental concern** — some developmental "concerns" reflect
environmental or opportunity factors rather than an intrinsic
developmental issue; the family-environment section exists to surface
  this distinction.
