# Ophthalmology Assessment

Ophthalmic evaluation covering visual acuity, anterior and posterior
segment examination, visual fields, and functional impact assessment.

## Scoring

**Instrument:** Visual Acuity Grade — graded by best-corrected visual
acuity level and functional impact.

## What it covers

Demographics; chief complaint; visual acuity; ocular history; anterior
segment; posterior segment; visual field and pupils; current
medications; systemic conditions; and functional impact.

## Common pitfalls

- **Recording visual acuity without noting whether it's corrected** —
best-corrected vs. uncorrected acuity are different measurements with
different clinical meaning; the distinction has to be explicit.
- **Missing systemic conditions relevant to eye findings** — several
systemic diseases (diabetes, hypertension, autoimmune conditions)
have characteristic ocular manifestations; skipping systemic history
can miss the underlying cause of an eye finding.
- **Treating functional impact as separate from clinical severity** —
measured acuity and functional impact on daily life don't always
track together (context, adaptation, and the specific visual task
  matter); both deserve independent documentation.
