# Occupational Therapy Assessment

Functional performance evaluation using the COPM (Canadian Occupational
Performance Measure) across self-care, productivity, and leisure
domains.

## Scoring

**Instrument:** COPM. Performance and Satisfaction each rated 1–10.

- **< 5** — significant issues.
- **5–7** — moderate concerns.
- **> 7** — good performance.

## What it covers

Demographics; referral information; self-care activities; productivity
activities; leisure activities; performance ratings; satisfaction
ratings; environmental factors; physical and cognitive status; and
goals and priorities.

## Common pitfalls

- **Scoring performance without also capturing satisfaction** — the
COPM's dual performance/satisfaction rating exists because a person
can perform a task adequately but be dissatisfied with how (or
whether) they do it — both numbers matter for setting meaningful
  goals.
- **Choosing activities to rate on the clinician's behalf rather than
client-identified priorities** — the COPM's design centres on the
client identifying which activities matter most to them; substituting
a standard checklist misses this client-centred structure.
- **Ignoring environmental factors** — a low performance/satisfaction
score can stem from environmental barriers (home layout, equipment
access) rather than the person's own capacity; the environmental
factors section exists to separate these causes.
