Demographics; menopause status; the MRS symptom scale; vasomotor symptoms; bone health; cardiovascular risk; breast health; current medications; contraindications screen; and treatment preferences.

Common pitfalls

• Skipping the contraindications screen because symptoms are severe — symptom severity doesn't override an absolute or relative contraindication (e.g. certain cancer histories, unexplained vaginal bleeding, active VTE); the screen has to be checked regardless of how bothersome the symptoms are.
• Treating MRS as covering all relevant risk — the scale measures symptom burden, not cardiovascular or breast-cancer risk directly; those require their own dedicated review even when the MRS score is high.
• Assuming one HRT regimen fits every eligible patient — treatment choice (systemic vs. local, combined vs. oestrogen-only) depends on whether the uterus is present, personal risk factors, and preference; eligibility alone doesn't determine the specific regimen.
