# Endometriosis Assessment

Endometriosis evaluation using revised ASRM (American Society for
Reproductive Medicine) staging and EHP-30 quality-of-life scoring, with
comprehensive pelvic pain profiling and fertility assessment.

## Scoring

**Instruments:** Revised ASRM Staging (points-based) and EHP-30 (0–100
per domain).

- **Stage I (Minimal)** 1–5 points; **Stage II (Mild)** 6–15; **Stage
III (Moderate)** 16–40; **Stage IV (Severe)** > 40.
- **Severity:** Mild (Stage I–II), Moderate (Stage II–III), Severe
(Stage III–IV), Critical (bowel/urinary obstruction or fertility
  crisis).

## What it covers

Demographics; menstrual history; pain assessment; GI symptoms; urinary
symptoms; fertility assessment; previous treatments; surgical history;
quality-of-life impact; and treatment planning.

## Common pitfalls

- **Assuming ASRM stage correlates with pain severity** — the ASRM
staging system is based on the extent and location of lesions seen at
surgery, and correlates poorly with reported pain severity in
practice; the EHP-30 quality-of-life score exists precisely because
stage alone doesn't capture patient-experienced impact.
- **Delaying assessment pending surgical staging** — ASRM staging
formally requires laparoscopic findings, but pain profiling, EHP-30,
and fertility assessment can and should proceed based on history and
non-surgical findings while awaiting or considering surgery.
- **Treating fertility assessment as a separate, later concern** —
endometriosis-related subfertility is common enough that fertility
assessment belongs in the same evaluation, not deferred until after
  symptom management is addressed.
