# Endocrinology Assessment

General endocrinology consultation assessment covering the thyroid,
adrenal, glucose, reproductive, pituitary, and bone/calcium axes,
integrating symptom review, clinical examination, and laboratory
findings to characterize endocrine disturbance and guide management.

## Scoring

**Instrument:** Endocrine Symptom & Biochemical Review (aligned to NICE
endocrinology guidelines). Per axis: Normal (indices within reference
range), Subclinical (biochemical abnormality without symptoms), or
Mild/Moderate/Severe (progressive symptomatic disturbance requiring
treatment escalation).

## What it covers

Demographics; presenting symptoms; thyroid axis review (TSH, FT4, FT3,
antibodies); adrenal axis review (cortisol, ACTH, aldosterone); glucose
metabolism (HbA1c, fasting glucose); reproductive axis (FSH, LH,
testosterone/oestradiol); pituitary function (prolactin, IGF-1, GH);
bone and calcium (PTH, vitamin D, calcium); medications and lifestyle
review; and clinical impression with a management plan.

## Common pitfalls

- **Reviewing one axis in isolation** — endocrine axes interact (e.g.
thyroid and reproductive hormones, or pituitary conditions affecting
multiple downstream axes); a single-axis focus can miss the actual
  underlying disturbance.
- **Treating "subclinical" as "no action needed"** — subclinical
biochemical abnormalities can still warrant monitoring or treatment
depending on the specific axis and patient context; the category
  isn't automatically benign.
- **Interpreting a single lab value without repeat confirmation** —
several endocrine tests (e.g. TSH, cortisol) are sensitive to timing,
stress, or acute illness; a single abnormal value often needs
  confirmation before acting on it.
