# Allergy Assessment

A comprehensive allergy evaluation covering drug, food, and environmental
allergies, with anaphylaxis risk assessment.

## Scoring

**Instrument:** Allergy Severity — a four-level categorical grade.

- **Low** — mild, localized reactions only.
- **Moderate** — systemic but non-life-threatening reactions.
- **High** — severe reactions or multiple allergen categories.
- **Critical** — history of anaphylaxis or life-threatening reactions.

## What it covers

Demographics; allergy history (onset, duration, patterns); drug
allergies with reactions; food allergies and intolerances;
environmental allergies (pollen, dust, mould, pet dander); anaphylaxis
history and emergency responses; testing results (skin prick, specific
IgE); current management (medications, immunotherapy, avoidance);
comorbidities (asthma, eczema, rhinitis); and quality-of-life impact
with a personalized action plan.

## Common pitfalls

- **Treating a single negative history as reassurance against future
  severe reaction** — allergy severity can escalate on re-exposure; a
  past mild reaction doesn't guarantee a mild reaction next time.
- **Omitting cross-reactivity between allergen categories** — food,
  drug, and environmental allergies can share cross-reactive mechanisms
  (e.g. latex-fruit syndrome); a category-siloed assessment can miss
  this.
- **No documented emergency action plan for at-risk patients** — anyone
  with an anaphylaxis history needs a clear, written plan (adrenaline
  auto-injector use, when to call emergency services), not just a
  severity label.
